Health and Medical Form Instructions 1. Campers must be
examined each year by a doctor and a dentist before camp. 2. Both Camper Health History Form 1 and Form 2 have to be up to date, and completely filled out. We will not permit any child to attend camp unless we have the required medical
documentation on file. a. A
current confidential medical history, including the child's immunization record
which shall include immunization dates against diphtheria, haemophilus
influenza type b, hepatitis b, measles, mumps, poliomyelitis, rubella, tetanus
and varicella (chicken pox), must be updated annually. b. Attach a photocopy
of the front and back of your health insurance card, and fill out the insurance
information on page one of Form 1. c. Sign the
Parent's Authorization in the boxes on the bottom of page 1. d. Your physician MUST complete and sign Form 2. 3. N.Y.S. Public Health Law requires us to maintain a
completed Meningococcal
Vaccination response form for
every camper who attends camp for seven (7) or more nights. Please complete the Meningococcal Vaccination Response Form and return it before June 1st. 4. Mail a completed Health Examination form promptly to: our
winter address if before June 1st. 5. If your child will be taking routine
medication at camp, you are required to
have your child's medication pre-packaged by CampMeds Inc. before coming to camp.
See the enclosed letter. 6. SUNSCREEN: The American Academy of
Pediatrics and the American Academy of Dermatology recommend sensible sun
protection as part of any health and fitness program. Accordingly, please remember to pack a good quality
sunscreen for your child. 7. HAIR CARE: Check your child's hair and scalp
carefully before their departure to camp, and follow-up with recommended
procedures for any necessary care and treatment (ask your Physician). |
